• All agreements involving the disclosure or exchange of information relating to University Intellectual Property to third parties must be forwarded to the General Counsel's Office for review, signature and record keeping. Please call them at 513-558-3485 with any questions.
   
• You must also use the appropriate Approval Sheets:
  • A910C - Confidential Disclosure Agreement Approval Cover Sheet  (Must be signed and submitted with any Confidential Disclosure Agreement to the IP Office for review and approval.)
  • A910M - Material Transfer Agreement Approval Cover Form (Must be signed and submitted with any Material Transfer Agreement (MTA) to the IP Office for review and approval.)

  • A910L - License Agreement Approval Cover Sheet (Must be signed and submitted with any Intellectual Property Licensing Agreement to the IP Office for review and approval.)

NOTE:  When filing Invention Disclosure Forms, please print out the Acrobat forms, fill-in in ink, and then forward the document to the UCIPO at M.L. 0829.  Do not send by e-mail.  In addition, please include any written description of the technology or manuscripts in electronic form (Word or WordPerfect for Windows).
